If your child is in 5th thru 8th grade, and plans to participate in sports activities for the 2017-2018 school year they must complete a sports physical with documentation and have it turned in to our Health Coordinator, Mrs. Guajardo, prior to the start of the season. We will be offering on site sports physicals on Saturday, August 19th, 2017 from 10:00 AM - 12:00 PM. Attached is the form that will need to be filled out.
